Submit a Report

Step 1: Let your organization respond anonymously Optional

To allow two-way anonymous communication with your company about your report, create a Reporting Account or log in to your existing account.

If you choose two-way communication, you may receive an email after you submit your report telling you the company has a new question. You can then log in to your Safe Hotline account to read the question and respond anonymously.

Only create an account if your organization is enrolled with Safe Hotline and you know its Company ID.

Don’t want an account? Go to Step 2. After you submit, you can use your Report Key on the Report Status (opens in a new tab) page to see if your company has responded. You won’t get email notifications, so you’ll need to check back on your own.

Step 2: Enter your report information

Required

Please provide as much detail as possible regarding the issue in the box below. Specific information will allow management to identify and address the issue. All information you provide will be included in your report.

If you wish to remain anonymous, please do not provide any self-identifying information in the details of your concern.

Step 3: Attach documents Optional

If you have documents to attach, click Select files, find the file on your computer, select it and click Open. Once the line turns orange and says 100%, your file has uploaded successfully. Files can be a maximum of 50 MB.

Step 4: Submit your report

After you submit, you’ll receive a Report Key. Keep it somewhere safe: you’ll need it to check your report status (opens in a new tab) or add information later.
